No-scalpel vasectomy is a simple surgical procedure for permanent male sterilization. The doctor uses a small puncture to reach the tubes ( vasa deferentia ) that carry sperm. These tubes are then cut and sealed to prevent sperm from mixing with semen. After the procedure, ejaculation still happens as normal but the fluid/semen no longer contains sperm .
-
Yes, ejaculation still happens as normal. The only difference is that the semen no longer contains sperm.
-
Most men feel only mild discomfort. Local anesthesia is used to numb the area.
-
Most men return to normal activities within few days, but it's best to avoid heavy lifting or strenuous exercise for about 10 days .

Possible Risks and Side Effects of Vasectomy
Although vasectomy is a safe, straightforward, and commonly performed procedure, as with any surgery, there are some risks and possible complications to be aware of. Most of these are rare and typically resolve with simple treatment or on their own.
1. Bleeding and Bruising
While uncommon, minor bleeding may occur during or shortly after the procedure. Occasionally, small internal bruises or blood collections (hematomas) can develop, though this is rare (approximately 1 in 1,000 cases). More commonly, minor bruising may appear on the skin of the scrotum and typically clears without intervention.
2. Infection
The risk of infection following vasectomy is low, but it can happen. Infections, if they occur, generally respond well to a short course of antibiotics.
3. Sperm Granuloma
A sperm granuloma is a small lump that may develop at the site where the sperm-carrying tubes were closed. It is a typical response to sperm leakage and usually does not cause serious problems. Some studies suggest its presence can even help lower pressure in the system and may improve the success rate if a reversal is ever considered.
4. Congestion or Inflammation
Some men experience a sensation of fullness or mild discomfort caused by a build-up of sperm in the system after the procedure. This may happen at any time following vasectomy and generally improves with the use of anti-inflammatory medication like ibuprofen. A small number of men (1–2%) may develop mild epididymitis (inflammation near the back of the testicle), which usually settles with conservative treatment.
5. Post-Vasectomy Pain (PVP)
Post-vasectomy pain is described as discomfort that lasts for several months or more and may interfere with daily activities. The occurrence of persistent pain is uncommon, and the severity can vary from low-grade, occasional discomfort to rare cases of persistent pain that may need further medical attention. In most situations, symptoms improve over time with rest and simple pain relief. It is estimated that 1-2 % of patients can have bothersome or persistent PVP, though we tend to see it much less frequently in our own patient experience .
6. Vasectomy Failure (Recanalization)
In rare cases, the sealed ends of the tubes can naturally reconnect, allowing sperm to pass again. This can happen early (within the healing phase) or later in life. Early failure is usually detected through routine semen analysis and may require the procedure to be repeated. Late failure is much less common, with long-term failure rates estimated at about 1 in 2,000 cases.

-
Vasectomy failure is extremely rare, but it is possible for the tubes to reconnect. This is why the follow-up semen test is essential to confirm that the procedure has worked. Early failure is usually detected during this follow-up test. Long-term failure is even more uncommon, with an estimated rate of about 1 in 2,000 cases.
